Form for using the right of withdrawal

I/we hereby give notice that I/we wish to withdraw from my/our contract for the purchase of the following goods:
- The contract was made on / the goods were received on (date)
- Name and address of the consumer
- Signature of the consumer (only if the form is sent on paper)
- Date
Returning the goods
The goods must be returned no later than 14 days after you gave notice that you are using your right of withdrawal. The goods should be in the same condition and quantity as when you received them. You may examine the goods in the same way as you would in a shop, but you may be liable for any loss in value caused by other handling.
